El Paso County Open Enrollment 2010
The month of October will be open enrollment for members covered under the Collective Bargaining Agreement in the CLEAT Dental and Vision Plan. During October you will be able to visit the CLEAT office during business hours and complete a change form to update your information. In addition, CLEAT representatives will be on site at the following dates and times
October 20, 2010 - Jail Annex 7:00 A.M. – 4:00 P.M.
October 21, 2010 - Downtown Jail – 7:00 A.M. – 4:00 P.M.
October 22, 2010 - SO Administration – 8:00 A.M. – 12:00 Noon
October 22, 2010 - CLEAT office 1:30 P.M. – 4:00 P.M.
• Changes will be effective December 1st, 2010
You will be able to:
• Verify current coverage
• Add or terminate coverage for dependents
• Change name
• Change address
• Update beneficiary / $15,000 AD&D policy included with each covered member
